How could a free market in health care bring lower costs?

Published June 06, 2012 in Short Answers by Mary Ruwart

QUESTION: Some people say that without all the government regulations on health care, the price of health care would drop. Could you explain this with a little more detail please?

MY SHORT ANSWER: Of course! Let’s use pharmaceuticals as an example.

In 1962, Congress passed the Kefauver-Harris Amendments to the Food and Drug Act, which greatly empowered the FDA. The time from the discovery of a new drug until it reaches the marketplace increased from 5 to 15 years; the cost of development increased to about $1.4 billion per new drug.

About 80% of what you pay at the pharmacy is due to the 1962 Amendments.

Any safety increases caused by this extra regulation were more than offset by the number of people who died waiting for life-saving drugs -- 4.1 million from 1962-2002.

Because of the increased cost, fewer new drugs were developed. Since it is much cheaper to take pills than to have surgery or hospitalization, the loss of these new drugs (about half of the innovations) means higher health care costs.

In other words, about 15-50% of current health care costs might have been saved without the 1962 Amendments, depending upon the assumptions that you make to calculate it. More importantly, somewhere between 4 and 16 million people died prematurely because these drugs never made it to market.

Regulations on health care providers and hospitals also drive up costs. Because the regulations are so numerous, it’s not so easy to calculate their costs. My guess would be that these regulations compromise our health, safety, and pocketbooks just as much as the 1962 Amendments.

For more detail, you may wish to read Chapters 5 and 6 of my book, Healing Our World, available from the Advocates (2003 edition) or as a free download (1993 edition) at my website, Ruwart.com.

How Lack of Consumer Choice Drives Up Health Care Costs” by Conn Carroll. This short blog entry from the conservative Heritage Foundation has a fascinating excerpt from a 2009 Atlantic magazine article. It points out that costs have dropped by 80% in Lasik surgery, which is not covered by insurance and where consumers shop and make informed decisions. That’s in sharp contrast to other medical services, the article points out.

Gravatar Don't forget the enormous tax burden your medical provider carries in the USA. High-earning medical providers frequently pay more than 50% of their income to various forms of local, state and federal taxation. If their tax burden were lowered, would it be possible that at least some of them might voluntarily lower their fees?

Gravatar The reason that health insurance is so expensive in this country is because half of our medical expenses are going to for-profit health insurance companies. They have a responsibility to maximize profits for their shareholders, thus, they make more money by finding ways of denying you health care and medications. To me that sounds immoral: Paying for medical insurance which may deny you treatment (generally the more crucial, the more expensive) and commonly practices litigation techniques to deny treatment in the hope that you will succumb to your ailment before they are forced to pay for your expensive regimen. This is the epitome of our false "free market." It's typical the new "Golden Rule": The one with the most gold makes the rules. During the health care debate, lobbyists from the insurance and pharma industries worked both sides of the aisle to successfully defeat the inclusion of a "government option" in "Obamacare." Lobbyists also funded many Tea Party "patriots" to attend town hall meetings and pose as concerned mothers, or just shout down any dialog about the advantages of different health care systems. It was like witnessing a murder in slo-mo. Meanwhile, in France, the annual per capita spending on health care is around $3600. In the US, it's around $7500. France's health care system is rated #1 globally by the World Health Organization. The US is rated #37, just below Cuba. Cuba, one of the poorest countries in the Americas, has a lower infant mortality rate than the US, and about the same life expectancy. Furthermore, health care cost crises are the second most prominent reason Americans default on their home loans. Many of the foreclosure "victims" have health insurance, and many have spotless credit ratings before their health emergencies. Losing one's home is psychologically and economically disastrous for the lives of the patients and their families, as well as their community small businesses and thus the economy in general. Can you picture having to choose between cancer treatment for a family member or your home? In Canada, no one has ever lost their home or life savings because of a medical crisis. No one. The Kefauver Amendment required drug advertising to disclose accurate information about side effects and efficacy of treatments. If you're a Libertarian and you don't happen to be a medical researcher, you might like to know that the medications you're about to give your children are considered safe and that you spent your money on something efficacious. Do you really want your family to be guinea pigs for Big Pharma? Do you really want the company paying for your health care to prioritize their shareholders over your well-being?

Gravatar Thalidomide. Europe gets their drugs out in 3 years and you end up with thalidomide. Whey not find out whats wrong with the system and fix it instead of throwing it out? Stop all illegal alien support (thats ILLEGAL not LEGAL). No hospital or doctor should be required to treat any illegal alien, ever for any reason. But we would need national ID system, to guarantee no mistakes... so pick your poisnon.
